Spring Grove Recovery ("we," "us," or "our") protects the privacy of individuals who visit springgroverecovery.com (the "Site"). This Privacy Policy describes how we collect, use, disclose, and safeguard your information in connection with our substance use treatment services.

1. Information we collect

We may collect the following when you visit or interact with the Site:

Information you give us. Name, email address, phone number, and anything else you choose to tell us — including health or treatment-related information — when you call admissions or submit the contact form on this Site. That form is hosted by JotForm under a Business Associate Agreement, which means it is contractually permitted to handle protected health information. Even so, keep clinical detail brief in a web form; a phone call is the better place for it, and admissions answers 24 hours a day.

Information collected automatically. IP address, browser type and version, pages visited, time and date of visit, and referring website, gathered through standard web technologies.

2. How we use your information

We use the information we collect to:

  • Respond to inquiries about our treatment programs
  • Provide support and guidance on admissions and coverage verification
  • Maintain and improve the Site
  • Comply with applicable laws and healthcare regulations
  • Communicate with you about our services, only where you have consented

3. Health information and HIPAA

Health information you share with us as a patient or prospective patient is protected health information, governed by HIPAA and — because we provide substance use disorder treatment — by the stricter federal confidentiality rule at 42 CFR Part 2. How we use and disclose that information is described in our Notice of Privacy Practices, which controls wherever it and this policy differ.

4. Disclosure of your information

We do not sell your personal information. We share it only with service providers who help us operate the Site and our admissions process under contract, and where required by law or to protect the safety of an individual.

5. Cookies, tracking, and the providers we use

We use cookies and similar technologies to operate the Site, remember preferences, measure traffic, and improve performance. This Site loads Google Tag Manager, which serves the tags listed below.

These third-party providers receive information about your visit:

  • Google Tag Manager — loads and manages the tags below.
  • Google Analytics — measures traffic and how visitors move through the Site: pages viewed, approximate location, device and browser, and the site you arrived from.
  • CallTrackingMetrics — changes the phone number shown on this Site depending on how you found us, so we can tell which sources produce calls. If you call a number shown here, the call may be recorded and the record kept.
  • TalkFurther — powers the chat window. Anything you type into chat is processed by that provider.
  • JotForm — hosts our forms on its HIPAA-covered service, under a business associate agreement.

These providers act for us under contract and may not use your information for their own purposes. Blocking cookies in your browser will not stop you calling us or otherwise using this Site.

Some pages on this Site have addresses that name a specific substance or condition, so a request from one of those pages can associate your device and network address with the subject you were reading. You should know that before you read on. If you would rather it not be measured, use your browser's tracking protection or a private window.

Our contact page loads no analytics. That page carries no tag manager at all. The contact form also appears on our home page, which does load Google Tag Manager — so a submission made from the home page is measured alongside that page, and one made from the contact page is not.

6. Your rights

Under the Colorado Privacy Act, Colorado residents have the right to access the personal data we hold, correct inaccuracies, request deletion, obtain a portable copy, and opt out of targeted advertising, the sale of personal data, and certain profiling. We honor a universal opt-out mechanism where your browser or an extension sends one.

Information about a health condition is sensitive data under the Act, and we do not process it for those purposes without your consent.

To exercise any of these rights, or to appeal a decision we make about a request, contact our Compliance Officer in Section 12. If we deny an appeal you may contact the Colorado Attorney General. Rights over your medical record specifically are covered by the Notice of Privacy Practices.

7. Security

We use industry-standard security measures to protect your information. No method of transmission over the Internet is completely secure, however, and we cannot guarantee absolute security.

8. Phone numbers on this Site

The number published on this Site rings our admissions team directly. We may add call-analytics tracking in future so we can tell which parts of this Site are useful; if we do, this section will be updated to say what is recorded and by whom before that happens.

9. Third-party links

The Site links to third-party websites, including external crisis resources and government pages. We are not responsible for the privacy practices of those sites.

10. Children's privacy

Our services are not intended for individuals under 18, and we do not knowingly collect personal information from minors without verified parental consent.

11. Changes to this policy

We may update this Privacy Policy periodically. The effective date above reflects the most recent revision.
